Student Teaching/Observation Placement Requests

Student Teaching/Observation/Practicum/Field Experience Placement Request Process:

  1. Those who wish to complete observation hours, practicum hours, student teaching or field experience in San Jacinto Unified School District should complete and submit the following form to request a placement: Observation/Practicum/Student Teaching/Field Experience Placement Request Form.  This form should be submitted at least 3 weeks prior to the requested start date for the placement.
  2. Proof of a negative TB test (taken within the last 2 years) will be required prior to beginning your placement.  This can be attached to the request form or emailed to Dr. Sharon Raffiee at [email protected].
  3. Upon receipt of the request, Human Resources will reach out to the appropriate site administrator and teacher to obtain approval.  If you wish to observe in more than one site, approval must be obtained for each site.
  4. Upon site approval, you will be notified if you need to set up an appointment to get fingerprinted.  (If you are a current Substitute employee or Volunteer, and have TB and fingerprints on file this step will not apply to you.)
  5. You will be notified by Human Resources of your placement once we receive proof of your negative TB test, site approval and fingerprint clearance (if required).

TB Test Information:

Proof of TB clearance is required.  You may obtain one from your own physician or any urgent care clinic.  Please call ahead for available times and pricing.

 

If you have a negative TB test result that is two years old or newer, please attach those results to your Placement Request Form or email them to Dr. Sharon Raffiee at [email protected].

 

Fingerprinting Information:

If you are notified that fingerprinting is required for the placement you are requesting, then please reach out to  Pamela Martinez at [email protected] to request the appropriate LiveScan form and a list of recommended places to go to complete this requirement.
