Transition Services


What are Transition Services?

Transition Services are services and activities that aid students with disabilities in their transition to life after school. These activities assess students' personal goals, interests, and needs, and assist students with their needs so they can find success in reaching their post-secondary goals. Transition Services include areas of:
  1. Instruction
  2. Related services
  3. Community experiences
  4. Development of employment and other adult living objectives
  5. Acquisition of daily living skills and functional vocation evaluation (if appropriate).
