2025-26 Waiver Form - Click Here!
In order to be eligible for any cash-in-lieu, you must 1) decline coverage online through Benefit Bridge, and 2) attach proof of other insurance and submit this completed form to the Benefits Office. 
**A new form is needed every year **

2024-2025 Insurance Information
 
 
Reminder, those waiving District sponsored medical insurance will need to complete a waiver form for the 2024-25 plan year, even if you have filled one out in the past.
A Waiver of Health Care Coverage form must be completed annually.
Waiver forms received for the 2024-2025 plan year will be paid out with the June 30, 2025 payroll.
