In order to be eligible for any cash-in-lieu, you must 1) decline coverage online through Benefit Bridge, and 2) attach proof of other insurance and submit this completed form to the Benefits Office. 
Waiver forms received for the 2025-2026 plan year will be paid out with the June payroll.
