2026-2027 Health Care Waiver Form
In order to be eligible for any cash-in-lieu, you must:
1) decline coverage online through Benefit Bridge AND
2) attach proof of other insurance and submit this completed form to the Benefits Office. 
Waiver forms received for the 2026-2027 plan year will be paid out with the June '27 payroll.
**Remember, a new form is needed every year **
