Shortly, you will receive a Parent Square notification which includes a downloadable verification form for you to upload during the last step of the application process. Please be sure to confirm your participation by verifying your child or dependent attends a “USDA Community Eligibility Provision (CEP) School” during the process. Although not required, you may also wish to have the last four digits of your Social Security Number and the last four of the SSN of your enrolled SJUSD student.
