From the selected families, only one (1) form per household needs to be completed. For example, a household or family with four (4) school age children attending different schools within San Jacinto USD should list all students on a single form. We cannot process an incomplete form, so families are asked to be sure to fill out all required information. Completed forms are to be given to a school office or dropped off at our Nutrition Services department (located at 221 Bissell Place, San Jacinto, CA 92583).
