Our goal is to ensure that every student is enrolled successfully in school and to support our families through that process.  We will be open all summer, Monday - Thursday and look forward to meeting you!

Head Start and State Preschool services are provided to qualified families. Children are admitted into the program based on priority using the Head Start or State Preschool Eligibility Criteria. Please contact the Preschool for more information on how to enroll your child.

ENROLLMENT PROCESSING APPOINTMENTS
Processing appointments are important for our Enrollment Technician to meet with you to review required information on enrollment packet forms for completion and required documents you provide.  Extra forms may be given to you especially related to student health and our dedication to meet your child's needs.
 
WALK-IN SERVICES
1) Enrollment Packets
  • We will review the checklist of what is required with you when you pick up an Enrollment Packet at the Enrollment Center.
  • We want to make sure that you know what is required with form completion and required documents.
  • Often, there are additional forms we can give you at that time, that will help you avoid an extra trip later
  • If you do not have all required documents at time of your appointment, unfortunately it will need to be rescheduled.

IMPORTANT RE; REQUIRED DOCUMENTS
  • It is important that you obtain the required documents and school records to provide for enrollment.
  • We are not able to process requests for required documents and records.
  • In the state of Californina, if you submit a Records Request to a previous district, they have 5 days to provide those records to the educational rights holder
 
ENROLLMENT PROCESS
1) Enrollment Packet:  Pick Up or Print from Website
2) Your Homework is to use the CHECKLIST on page 2 of the enrollment packet and
    A) Complete Each Form
    B) Collect ALL Required Documents
3) Make an Appointment:  Needed to complete enrollment with an Enrollment Technician
4) Enrollment Appointment Meeting:
    • Arrive on time
    • Bring completed enrollment packet
    • Bring ALL required documents
    • Sign-in on Computer
    • Put Enrollment Packet and ALL required documents in the basket
    • Turn basket into receptions
 
ENROLLMENT REQUIREMENTS
KINDERGARTEN
  • Transitional Kindergarten:
        -- 2 year kindergarten program
        -- Child's birthdate must fall between September 2 and December 2 of the year they enter school
  • Kindergarten:
        -- Child must be 5 years old by September 1 of the year they enter school

 

VERIFICATION OF PARENT/GUARDIAN IDENTITY

Any one of the following items (with picture) is acceptable verification of identity:

  • Current Driver’s License
  • California Identification Card
  • Passport
  • Military ID
  • Bank Identification Card
  • Company/Work Identification

The following documents qualify as verification:

  • Birth Record (Includes:  Hospital Birth Record, County Recorder Verification, Adoption Records, Foster Care Records)
  • Birth Certificate
  • Baptismal Certificate
  • Passport
  • Affidavit

 

verification OF RESIDENCY

One form of verification of residency must be provided.  The following items will serve as verification and must be current (dated within the last 30 days):

  • Utility Bill (Electric, Gas, Water, Trash, Mortgage)
  • Government Form:  State or Federal
  • Pay Stub
  • Bank Statement
  • Lease or Rental Agreement
  • Final Escrow Papers

 

VERIFICATION OF IMMUNIZATIONS

Proof of the following immunizations must be provided and proper intervals between doses must be observed, or additional immunizations will be required before admission. See chart in enrollment packet for immunization requirements.

July 1, 2019:  TK/K - 12 STUDENTS NEED THE FOLLOWING TO BE ENROLLED 

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) - 5 Doses
      4 Doses OK if one was given on/after 4th birthday / 3 Doses OK if one was given on/after 7th birthday
         Grade 7-12: 1 Dose with Pertussis on/after 7th birthday
  • Polio (OPV or IPV) - 4 Doses
       3 Doses OK if one was given on/after 4th birthday
  • Hepatitis B - 3 Doses
      Grade 7:  NOT REQUIRED
  • Measles, Mumps, and Rubella (MMR) - 2 Doses
      Both given on/after 1st birthday
  • Varicella (Chickenpox) - 2 Doses
      IMPORTANT:  If child had the disease/Chickenpox - they still need one of the following:
      A)  2 Doses of Varicella
      B)  Medical Exemption
            This is a WRITTEN STATEMENT from a physician (MD or DO) licensed in California that states each of the following:
      •  The specific nature of the physical condition or medical circumstance of the child for which a licensed physician does not recommend immunization
      • Eachspecific required vaccine that is being exempted
      • Whether the medical exemption is permanent or temporary 
      • The expiration date, if the exemption is temporary
 

PHYSICAL EXAMINATION

Students enrolling for kindergarten and first grade must show proof of physical examination that includes the required blood, urine, vision and audiometric testing. 

  • The exam for kindergarten must be dated no earlier than 6 months prior to the first day of attendance at school and at the time of the exam, the child must be at least 4 years and 3 months old.
  • To enroll in first grade, the exam must have been given within 18 months prior to the first day of attendance at school.

 

ORAL HEALTH ASSESSMENT

Students enrolling for the first time in public school, in kindergarten or first grade are to have a dental check-up by May 31st of the first school year. An evaluation within 12 months prior to school entry will meet this requirement.

Oral health education materials and the assessment form to be completed by a licensed dental professional will be provided to parents who are enrolling their children for public school for the first time, in either kindergarten or first grade.
